Pain Relief: Ice Or Heat?!? | Seniors Flourish

You never forget your first gout attack. The severe pain, swelling, and redness hits hard and fast. The initial attack often strikes your big toe’s large joint, but later ones might affect the foot or ankle. Other hot spots include the knees, hands, and wrists.

First-line treatment is quite effective and includes anti-inflammatory medications, ice therapy, and rest. A combination of diet and lifestyle changes and prescription drugs an approach called urate-lowering therapy, or ULT is typically recommended if attacks recur or become more severe.

Which Is Better For Relief Of Gout Pain Hot Or Cold Soaking

By | Submitted On May 12, 2010

When it comes to pain, there are always 2 ways to help relief the pain, ice bag or warm tower. Somehow, the cold treatment is a more commonly used solution to numb the nerves so that the signals of the pain cannot reach the brain, temporarily.

In the case of gout pain, because the infected area is like burning on fire, most patients will soak the whole foot into a bucket full of freezing cold ices. That will immediately numb all the nerves in the foot, he cannot even feel the existence of the feet, not to mention the grueling pain at the swollen toe.

This may sound like a good fix for the intense pain, but the fact is that cold treatment will only make the condition worse in the long run. This is a method that kills the symptoms but worsen the root cause of gout.

Gout is a direct result of accumulation of too much uric acid in the body and the population of gout patients is growing worldwide at alarming rate. This is because of our modern stressful lifestyle that is full of acidic diet and habits. Smoking, soft drinks, instant food, synthetic food additives, fast food, are all acidifying agents that actively pump acids into the bloodstream.

When the concentration of uric acid in the synovial goes beyond the saturation level, they will crystallize and forms urate crystals that triggers the immune reaction which result in inflammation, burning sensation and pain, intense pain! Gout attack!

Does A Heating Pad Help With Nerve Pain

The combination of hot and cold increases the circulation of fresh blood to the area, which may help relieve pain. Hold an ice pack over the affected area for about 15 minutes at a time, three times a day to help reduce inflammation. Heat pads can be applied for a longer period, up to 1 hour, three times a day.

Ice packs for pain relief: how to use them for sprained ...

The American College of Rheumatology released gout management guidelines in 2012 and updated them in 2020. They recommend drugs classified as urate-lowering therapy for those who have experienced two or more gout attacks in a year as well as for those with joint damage from gout.

Specifically, the organization recommends allopurinol as a first-line treatment for all gout patients, even those with chronic kidney disease. Next in line is febuxostat both are a class of drugs called xanthine oxidase inhibitors.

In addition, the guidelines recommend use of anti-inflammatory medications including NSAIDs, prednisone, or colchicine along with the other medication.

Heat Therapy And Chronic Gout

The heating pad treatment for gout is best with the chronic type. The chronic gout attacks are characterized by less sharper pains than those of acute attacks. Although it may not dramatically relieve the pain with the more painful acute gout attacks, heat therapy may work well in conjunction with other forms of treatments such as medications during these attacks.

Why Do I Suffer From Gout

One word: hyperuricemia. Or, in other words, increased uric acid in your blood. This is the reason why your joints are painful and inflamed. Uric acid is a normal product in your body it is a substance that breaks down purines in your body which are found in certain foods that we eat, in addition to being found in human cells. When your body overproduces uric acid, youre at risk for developing gout. But, why would your body overproduce uric acid?

The triggers of hyperuricemia include:

  • Injury
  • Excessive alcohol consumption
  • Eating large amounts of red meat and shellfish
  • Dehydration
  • Drinking sugary sodas

It is these types of triggers that can create the hyperuricemia, which can result in gout. When you overproduce uric acid, your kidneys have to work harder to flush out the extra uric acid out of your bloodstream, as they would normally do.

What About Injured Muscle Muscle Strains

If youre supposed to ice injuries, but not muscle pain, then what do you do with injured muscles ? That can be a tough call, but ice usually wins but only for the first few days at most, and only if it really is a true muscle injury. A true muscle injury usually involves obvious trauma during intense effort, causing severe pain suddenly. If the muscle is truly torn, then use ice to take the edge off the inflammation at first. Once the worst is over, switch to heat.

How Does Ice Help Gout Pain

Should I use ICE or HEAT for my PAIN?

Hot and cold therapy is something of a routine treatment nowadays for many types of pain. The theory is that while cold therapy helps to reduce inflammation and swelling, the heat pads then relax your muscles.

When dealing with gout however there is no muscular pain as such that needs to be treated. But there is most certainly plenty of painful inflammation and swelling so cold therapy and more specifically ice is often recommended for gout.

The benefits of applying ice to help gout could be summarised as follows

1) Reduced Swelling

The ice is designed to provide a cold sensation to the joint or area to which it is applied. This cold causes the blood vessels around the joint to constrict, thus reducing blood flow and pressure around the affected joint.

2) Synovial Fluid Reduction

Too much synovial fluid is sometimes over-produced in response to the build-up of urate crystals, and can actually add to the swelling or pain of an attack .

3) Numbing Effect

Applying ice to an area of skin has been known to provide a numbing effect, both on the skin and just underneath.

Again, this can help with pain reduction, especially when gout makes the skin around the affected joint super-sensitive.

4) Distraction

While gout may be the cause, it is your brain that ultimately receives and process the pain signals, before turning them in to actual feelings.

Poorly sent pain signals means they wont be processed correctly and your pain will be less than otherwise expected!

What Ice And Heat Are For

Ice is for fresh injuries, and heat is for stiff, aching muscles. Roughly. But the devil is in the details, and there are a lot of them.

Hasnt icing been myth-busted? Its been fashionable lately to debunk icing.1 The backlash against icing has some merit, and I love to see myths busted of course, but I think in this case its mostly a non-issue. The majority of icing is done for minor pain control, and theres really no problem with that.

Ice is for injuries calming down damaged superficial tissues that are inflamed, red, hot and swollen. The inflammatory process is a healthy, normal, natural process that also happens to be incredibly painful and more biologically stubborn than it needs to be. Icing is mostly just a mild, drugless way of dulling the pain of inflammation and taking swelling down a bit we hope.2 Examples: a freshly pulled muscle or a new case of IT band syndrome .

Heat is for muscles, chronic pain, and stress taking the edge off symptoms like muscle aching and stiffness, which have many unclear causes,3 but trigger points are probably one of the usual suspects.4 Chronic pain, especially back pain, often involves lots of tension, anxiety, hypervigilance, and sensitization, and comfortable heat can soothe a jangled mind and nervous system. Stress and fear are major factors in many painful problems, of course.

Pain From Sciatica Can Range From Mild To Intense

Pin on Gout Massage Magazine

Dear Mayo Clinic:

Ive had sciatica on-and-off for weeks, but for the past few days it has been so bad that Im uncomfortable all of the time. It feels best when walking, but when I stop or sit down the pain is almost unbearable. My doctor says physical therapy is the only way to treat it. Are there no other options?

Answer:Sciatica refers to pain that radiates along the path of the sciatic nerve, which runs from the lower back to the buttock and down the back of each leg. The lower lumbosacral nerve roots make up the sciatic nerve which is the longest nerve in the body.

Although many people think of sciatica as a condition, it is actually a collection of symptoms. Most people with sciatica will experience pain more in their lower extremity than in their back. The pain will often extend below the knee and into the foot and, in many cases, it is accompanied by numbness or tingling. On occasion, patients will experience muscle weakness in the affected leg.

For some, sciatica pain may be just a mild ache for others, it can be quite intense. Some people experience a sharp burning sensation and may feel a jolt like an electric shock. Often, the pain intensifies when sitting for long periods. Some patients lose bladder or bowel control, a sign of cauda equina syndrome, which is rare but serious and requires immediate medical care. See your doctor immediately if your sciatica is accompanied by these symptoms.

When Is Surgery Considered For Gout

The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone. Whenever possible, however, we try to avoid surgery to remove tophi. The problem is that the crystals are often extensive, and track back to the bone, so there is not a good healing surface once the tophus is removed. In some rare cases, such as when a tophus is infected or when its location is causing major disability, surgical removal may be considered.

Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase , since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus.

What Ice And Heat Are Not For

Both ice and heat have the potential to do some minor, temporary harm when used poorly. Heat can make inflammation significantly worse. Ice can aggravate symptoms of tightness and stiffness it can also just make any pain worse when its unwanted.

Both ice and heat are pointless or worse when unwanted: icing when youre already shivering, or heating when youre already sweating. The brain may interpret an excess of either one as a threat, but icing is more threatening and when brains think theres a threat, they may also amp up the pain.5 Ice seems to be feel more threatening to most people.

Be especially wary of icing muscle pain and it may not be obvious. You may think your back is injured, for instance, but it may just be muscle pain. Trigger points can be surprisingly intense and easily mistaken for iceable injury and inflammation. But if you ice trigger points, they may burn and ache even more acutely. This mistake is made particularly often with low back pain and neck pain the very conditions people often try to treat with ice.

Heat and inflammation are the other particularly bad combination. If you add heat to a fresh injury, watch out: its going to get worse! A physician once told my father to heat a freshly injured knee, and wow it swelled up like a balloon, three times bigger than it had been before. And three times more painful.6

Whats The Best Way To Take Lab

Should you use heat or ice for joint pain?

Your body doesnt typically excrete enough uric acid on its own to minimize the risk of a gout attack. Lab-grade Chanca Piedra is that extra added push for your body to be able to not only flush out uric acid in the kidney and liver, but to also inhibit the formation of uric acid crystals. Chanca Piedra has also been shown to help with inflammation.

To dissolve uric acid crystals or to prevent them from forming altogether:

  • Take 400mg of lab grade chanca piedra when you wake.
  • Wait one hour before eating to ensure maximum uptake of the herb.
  • Take 400mg of lab grade chanca piedra at night.
  • Once your gout attacks are under control, scale back to just 400mg per day.
  • Keep in mind that each individual is unique and will respond differently to each treatment type. Always discuss any new treatments that you are considering with your physician prior to starting them so he/she can review with you any potential medication interactions and potential side effect concerns.
