Can You Recover From Gout

Be Choosy About Carbs

Gout Diagnosis

Carbs can be helpful or harmful to your gout health. The most helpful carbs are found in whole grains, fruits, vegetables, and legumes. Sweet potatoes, beans, apples, and popcorn can all be served in wholesome and delicious ways as part of a healthy gout diet.

Other carbs come as natural and refined sugars. Candy, sports drinks, some breakfast cereal, and some pasta sauces all contribute to dietary sugar from unhealthy carbs.

How Is Gout Treated

Gout can be effectively treated and managed with medical treatment and self-management strategies. Your health care provider may recommend a medical treatment plan to

  • Manage the pain of a flare. Treatment for flares consists of nonsteroidal anti-inflammatory drugs like ibuprofen, steroids, and the anti-inflammatory drug colchicine.
  • Prevent future flares. Making changes to your diet and lifestyle, such as losing weight, limiting alcohol, eating less purine-rich food , may help prevent future attacks. Changing or stopping medications associated with hyperuricemia may also help.
  • Prevent tophi and kidney stones from forming as a result of chronic high levels of uric acid. Tophi are hard, uric acid deposits under the skin. For people with frequent acute flares or chronic gout, doctors may recommend preventive therapy to lower uric acid levels in the blood using drugs like allopurinol, febuxostat, and pegloticase.

In addition to medical treatment, you can manage your gout with self-management strategies. Self-management is what you do day to day to manage your condition and stay healthy, like making healthy lifestyle choices. The self-management strategies described below are proven to reduce pain and disability, so you can pursue the activities important to you.

Home Remedies For Long

The American College of Rheumatology recommends a couple of lifestyle modifications for preventing future attacks of gout:

1) Lose weight if you need to. Being overweight can increase uric acid levels as well as put pressure on the joints.

2) Follow a low-purine diet. Another effective way to reduce uric acid levels is to follow a low-purine diet. Purines are organic compounds that break down into uric acid. Following a low-purine diet means avoiding the big four alcohol, shellfish, red meat, and high fructose corn syrup.

Limiting these foods is also beneficial for heart health and people with gout are at higher risk for heart disease.

But dont go overboard with dietary changes to relieve gout pain though, advises Dr. Fields. You can find lists of thousands of different foods youre not supposed to have if you have gout. Those recommendations are impossible to follow and can make you crazy.

Easing Back Into Walking And Exercise After A Gout Flare

After a gout flare subsides, Dr. Iversen suggests aquatic exercises may be a good way to start re-engaging in exercise because the buoyancy of the water will reduce the impact on the joints.

Low-impact aerobic exercises can be helpful, too, such as on an elliptical machine. She says its important to keep your joints flexible by incorporating stretching and range-of-motion exercises once the gout flare subsides to promote good joint movement.

Be careful not to overdo it once you ease into post-flare exercise, she cautions. Patients should not be experiencing pain when walking once the flare has subsided. If you do experience pain with walking after a flare, go back to using a walking support and reduce your planned exercise until the pain subsides.

Stage : Intercritical Gout

After a first gout flare, 75 percent of people will have a second within a year but some people can go years before another attack, says Dr. Fields. The in-between stage is where a person has already had a gout flare but is presently not having any joint pain or swelling, he says. Almost all gout patients will go through this phase, since it is the nature of gout to have flares and then quiet down for a period of time before the next flare.

Even though it may seem like nothing is happening, this is the point in which patients should begin long-term treatment. Lowering uric acid levels with medication can prevent future gout flares and long-term complications that go with them.

When Is Surgery Considered For Gout

The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone. Whenever possible, however, we try to avoid surgery to remove tophi. The problem is that the crystals are often extensive, and track back to the bone, so there is not a good healing surface once the tophus is removed. In some rare cases, such as when a tophus is infected or when its location is causing major disability, surgical removal may be considered.

Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase , since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus.

Avoid Sweets And Sodas

Unlike fruit with its natural sugar, artificial sugars are widely recognized as gout flare dangers. Consuming too many sugary sodas has been found to provoke attacks, but no association has been found with artificially sweetened sodas. Other foods loaded with high-fructose corn syrup can also bring on gout pain.

How Does A Doctor Diagnose Pseudogout

Pseudogout is suggested when abnormal calcifications are seen in the cartilage of joints on X-rays. These calcifications are referred to as chondrocalcinosis and very suggestive of a diagnosis of pseudogout when there is also inflammation of the involved joint.

The diagnosis of pseudogout is ultimately made when fluid from a joint is extracted and is examined under a special microscope called a polarizing microscope. With this microscope, the calcium pyrophosphate crystals are clearly identified by their characteristic shape and color .

The arthritis of pseudogout is common in older adults, particularly in the context of dehydration such as occurs with hospitalization or surgery.

The treatment of pseudogout is directed toward stopping the inflammation in the joints. Local ice applications and resting can help. Nonsteroidal anti-inflammatory drugs such as ibuprofen and others are often first drugs of choice.

Removing fluid containing the crystals from the joint can reduce pain and help the inflammation to diminish more quickly. Cortisone injected into an inflamed joint will also help decrease the inflammation. Colchicine is a medication used to decrease inflammation in patients with gout and may also be helpful in pseudogout. Corticosteroids taken as a pill can help if the patient can’t tolerate colchicine or NSAIDs.

Long-term prevention of recurrent pseudogout is often best achieved with small daily doses of colchicine and optimal hydration.

Arthritis / Acute Gout Attack

5 Natural Remedies for Bone and Joint Pain

Gout is a form of arthritis, hence it causes pain and discomfort in the joints. A typical gout attack is characterized by the sudden onset of severe pain, swelling, warmth, and redness of a joint. The clinical presentation of acute gouty arthritis is not subtle with very few mimics other than a bacterial infection.

The joint most commonly involved in gout is the first metatarsophalangeal joint , and is called podagra. Any joint may be involved in a gout attack with the most frequent sites being in the feet, ankles, knees, and elbows.

An acute gout attack will generally reach its peak 12-24 hours after onset, and then will slowly begin to resolve even without treatment. Full recovery from a gout attack takes approximately 7-14 days.

An accurate and colorful discription of a gout attack was elegantly written in 1683 by Dr. Thomas Sydenham who was himself a sufferer of gout:

Gout is common but widely misunderstood.

There is a misconception that it is directly linked to alcohol consumption and gluttony, but diet is more likely to be a trigger for an attack rather than the sole cause.

Although dietary changes alone cannot get rid of gout, adapting what you eat can boost wellness and help with living with the condition.

And remember, it is a serious, chronic condition, so always follow the advice of a doctor on how to treat gout.

The Role Of Diet In Gout Prevention

Dietary control may be sufficient in a patient with mildly elevated uric acid, for example, 7.0 mg/dL

For those with a higher level, for example, 10.0 mg/dL, diet alone will not usually prevent gout. For the latter, even a very strict diet only reduces the blood uric acid by about 1 mg/dL- not enough, in general, to keep uric acid from precipitating in the joints. The cutoff where patients with gout seem to dramatically reduce their number of attacks is when their uric acid level is taken below 6.0 mg/dL.4

Help Prevent Gout Attacks:

Understanding Gout // Causes, Triggers and Treatments
  • Do not eat high-purine foods. These foods include meats, seafood, asparagus, spinach, cauliflower, and some types of beans. Healthcare providers may tell you to eat more low-fat milk products, such as yogurt. Milk products may decrease your risk for gout attacks. Vitamin C and coffee may also help. Your healthcare provider or dietitian can help you create a meal plan.
  • Drink liquids as directed. Liquids such as water help remove uric acid from your body. Ask how much liquid to drink each day and which liquids are best for you.
  • Maintain a healthy weight. Weight loss may decrease the amount of uric acid in your body. Ask your healthcare provider what a healthy weight is for you. Ask him or her to help you create a weight loss plan if you are overweight.
  • Control your blood sugar level if you have diabetes. Keep your blood sugar level in a normal range. This can help prevent gout attacks.
  • Limit or do not drink alcohol as directed. Alcohol can trigger a gout attack. Alcohol also increases your risk for dehydration. Ask your healthcare provider if alcohol is safe for you.

Untreated sleep apnea can increase the risk of gout.12,13 Sleep apnea is a sleep disorder that causes the body to take in less oxygen while sleeping, which in turn causes the body to produce more purines.12,14 Sleep apnea is typically treated with a continuous positive airway pressure machine or other device that increases oxygen intake while sleeping.

Pain Relief Without Medicine

Use cold. If your pain isn’t too bad, try cold packs or compresses on the joint to lower inflammation and soothe the ache. Wrap ice in a thin towel and apply it to the joint for up to 20 minutes several times a day. Do not apply ice to your hands or feet if you have nerve problems from diabetes or other causes.

Rest the joint. It’s a good idea to rest it until the pain eases up. You probably won’t want to move it much anyway. If you can, raise the joint on a pillow or other soft object.

Drink water. When your body doesnât have enough water, your uric acid levels rise even higher. Stay hydrated to help keep those levels normal.

Watch what you eat and drink. Foods that are high in substances called purines, such as some seafood, organ meats like liver, and fatty foods, can raise the uric acid in your blood even more. So can fructose-sweetened drinks and alcohol — especially beer.
