How Alcohol Affects Gout
Drinking alcohol affects gout risk in two main ways:
Alcohol makes the uric acid be pulled back into the body, with less in the urine. This leads to elevated blood levels of uric acid, says Theodore R. Fields, MD, a rheumatologist at the Hospital for Special Surgery in New York City. When uric acid isnt excreted in urine, it further contributes to the formation of crystals in joints that cause intense pain and inflammation, says Jonathan Greer, MD, a rheumatologist with Arthritis & Rheumatology Associates of Palm Beach, Florida.
An analysis of multiple studies on alcohol consumption and gout which included data on almost 43,000 people found a dose response of gout risk to alcohol consumption. That means that the more alcohol people consumed, the greater the risk of developing gout.
Compared with people who never or rarely consumed alcohol, light drinkers had a 16 percent increased risk of gout, moderate drinkers had a 58 percent increased risk of gout, and heavy drinkers had a 264 percent increased risk of gout.

Which Type Of Alcohol Is The Worst For Gout
All types of alcohol affect gout, but the impact on flares and symptoms may vary by type of alcohol, depending on which studies you look at. Some research suggests that beer is especially bad for gout because it contains higher levels of purines that break down directly into uric acid.
A 2004 study published in the medical journal The Lancet found that alcohol is strongly associated with an increased risk of gout, the study authors concluded. Risks varied according to type of alcoholic beverage:
- Two or more beers daily increased gout risk over non-beer drinkers two-fold
- Two shots of spirits daily increased gout risk over non-drinkers by 1.6 times
- Two four-ounce glasses of wine daily was not associated with a higher risk of gout
However, other research has found a link between wine consumption and gout risk. A study published in the American Journal of Medicine in 2014 looked at how much alcohol consumed over a 24-hour period was associated with a recurring gout attack. It found that all types of alcoholic beverages, including wine, were associated with an increased risk for recurrent gout attacks. Even though one drink didnt raise the subjects risk for a gout attack by that much, having one to two drinks in a 24-hour period was associated with a 36 percent higher risk of recurrent gout attack, compared with those who had consumed no alcohol in that time period.

Soft Drinks Worse Than Hard Liquor for Gout, but Diet Sodas OK
Jan 31, 2008 — For gout, sweetened soft drinks are worse than hard liquor — and nearly as bad as beer — doubling the risk for heavy drinkers. Diet sodas, however, don’t affect gout risk.
The findings come from a huge study of 46,393 male health professionals in Canada who filled out detailed questionnaires about their health and their diet every four years for 12 years. Over that time, 755 of the men developed gout.
Compared with men who almost never drank sugar-sweetened soft drinks — fewer than one per month — frequent soft-drink drinkers were significantly more likely to suffer gout:
- Two or more soft drinks each day upped gout risk by 85%.
- One soft drink each day upped gout risk by 45%.
- Five or six soft drinks each week upped gout risk by 29%.
The men who drank the most soft drinks had twice the gout risk of the men who drank the fewest soft drinks.
That’s comparable to the gout risk of men who drink two to four alcoholic beverages a day.
Beer raises gout risk by 49% per daily serving. A daily serving of spirits raises gout risk by 15%. Sweetened soft drinks, find University of British Columbia researcher Hyon K. Choi, MD, PhD, and colleagues, raise gout risk by 35% per serving.
It’s a surprising finding, says Karen Atkinson, MD, MPH, chief of rheumatology at the Atlanta VA Hospital and assistant professor of medicine at Emory University.

Soft drinks ‘bigger gout risk than alcohol’ read the headline in The Daily Telegraph today. It reported that drinking too many sugary soft drinks and fruit juices can substantially increase the risk of gout. Gout can be extremely painful and is caused by a build up of uric acid that crystallises in the joints. Uric acid is formed by the breakdown of purines which occur naturally in the body as well as in the diet. Traditionally, the advice is to avoid red meat and alcohol as they have high levels of purines and can make gout worse. However the Telegraph said the risks associated with these drinks were higher than with certain types of alcohol.
This story is based on a well-designed study in more than 46,000 men that found those who drank two or more cans of soft drinks a day increased their risk gout by 85 per cent compared with men who drank less than one soft drink a month. It provides yet another reason why drinking sugary fizzy drinks is not good for your health.

Other Definitions Of Dietary Exposure And Gout
When analyses were repeated using baseline dietary intake and updated dietary intakes every four years without cumulative averaging, the results remained significant. The multivariate relative risk between the extreme fifths of free fructose substituting for other carbohydrates with baseline dietary intake was 1.81 and with updated information without cumulative averaging was 1.93 . The corresponding multivariate relative risks for substituting free fructose for fat were 1.59 and 1.77 .
With other case definitions of gout, the magnitudes of associations tended to increase as specificity of the case definition increased, but null associations remained null. For example, as the definition became more specific, going from self reported gout , to gout defined by the American College of Rheumatology criteria , to tophaceous or crystal proved gout , the multivariate relative risks between the extreme fifths of free fructose intake substituting for other carbohydrates were, respectively, 1.65 , 2.02 , and 2.25 . The corresponding multivariate relative risks for substituting free fructose for fat were 1.54 , 1.81 , and 2.23 .
Be Choosy About Carbs
Carbs can be helpful or harmful to your gout health. The most helpful carbs are found in whole grains, fruits, vegetables, and legumes. Sweet potatoes, beans, apples, and popcorn can all be served in wholesome and delicious ways as part of a healthy gout diet.

In a word, no. Eliminating or cutting back on alcohol alone likely wont lower uric acid levels enough to effectively treat gout. For many people with gout, the target uric acid level is less than 6 mg/dL. If a person with high levels of uric acid goes on a diet, loses weight, eliminates high-purine foods like shellfish and stops drinking beer, they can lower their uric acid from, say, 10 to 9, but usually not much lower. Dietary improvements are still not enough the patient has to be on uric acid-lowering medication at this level, says Dr. Fields.
It is a myth that gout is a dietary disease, and that watching your diet will be enough to manage gout.
Gout is a genetic disease and extremely few people can make enough of a change in their blood uric acid level with diet to control their gout, says Dr. Fields. That said, its still important for people to limit those foods and beverages mentioned as part of their treatment, especially in the first six months after they start uric acid-lowering therapy when the patient is vulnerable to flares since these foods and drinks can make gout worse. Watching your diet, especially early in treatment, can make a difference, says Dr. Fields.
Are Lemon Juice And Baking Soda Helpful For Gout
What you eat and don’t eat is an important part of the management plan for those with gout. While there is some preliminary evidence that getting more vitamin C in your diet from foods such as lemon juice may help, it’s not a cure for the painful arthritis, and baking soda may worsen your condition.
