Hallux limitus is the initial stage of a gradual onset of arthritis in the big toe, which causes the range of motion in the big toe to be limited.
Hallux rigidus is used to describe the later stages, where the big toe becomes rigid and difficult to move or flex. Both hallux limitus and rigidus are caused by the decomposition of cartilage around the joint of the big toe, which causes pain and limited range of motion.
Although the pain can feel similar to gout, gout is a type of inflammatory arthritis that occurs when uric acid found in the blood stream gets deposited and becomes crystalized in the joint at the base of the big toe.

Younger Gout Patients Have Higher Blood Clot Risk

MONDAY, June 3, 2019 — Older age raises the odds of many ills, but for adults with gout, it’s the younger ones who have the highest risk for developing a serious blood clot, new research indicates.
Gout patients of any age have a 25% greater risk of developing a blood clot deep in the veins in the first 10 years after diagnosis, the British study found.
But “the risk was 79% higher in gout patients, compared to those without gout, in the under-50 age group,” explained study lead author Alyshah Abdul Sultan.
Gout is the most common form of inflammatory arthritis, affecting more than 4% of U.S. adults, according to the Arthritis Foundation. It develops in people who have high levels of uric acid in the blood. The acid can cause painful crystals to form in the joints.
In this study, researchers compared roughly 62,000 gout patients with an equal number of gout-free adults. The investigators found that the raised blood clot risk was largely confined to gout patients under the age of 50.
“We did not observe much higher risk in the older population,” added Sultan, a fellow at Keele University’s arthritis research center in Staffordshire, England. But, he noted, because blood clot risk increases with age regardless of gout status, “it may have obscured the effect of gout in the older population.”
“As the absolute increased risk is small, the presence of gout alone would not warrant use of anticoagulation therapy,” Fonarow said.

Study Design And Patient Enrollment
This study was a retrospective study on a total of 50 patients with gout flare and 27 patients with gout in remission phase. All of the patients had been treated at the Department of Rheumatology, The First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 1st, 2013 through October 31th, 2014. Gout was diagnosed in the patients following the criteria of American College of Rheumatology established in 2015 . Patients with gout flare were diagnosed if she/he had extreme tenderness and pain in the joints within hours or 1-2 days with desquamation of the skin around the joints. Patients with gout in remission were defined if they had UA level of < 6 mg/dL in two consecutive measurements and no acute attack with tolerable pain score of < 2, as well as no tophus during the last 12 months. In addition, patients had normal ranges of serum, urine creatinine and urea. Patients, who had gout were excluded from this study if the patient had hypertension, acute or chronic nephritis, diabetes mellitus, urinary infection, cardiac failure or cardiovascular disease, recent nephrotoxic drug use or an infectious disease. Informed consent in written form was obtained from all the patients. The study protocol was approved by The Institute Review Board of The First Affiliated Hospital of Guangzhou University of Chinese Medicine.

If you have chronic kidney disease and your creatinine clearance is at least 15 mL/min, its fine to take Xarelto to treat or prevent blood clots such as deep vein thrombosis * and pulmonary embolism .** But if your creatinine clearance is lower than this, Xarelto isnt usually recommended.
If you have chronic kidney disease, and youre taking Xarelto to reduce the risk of heart attacks and stroke, its fine to take the drug regardless of your creatinine clearance.
