What Is Gout And What Causes It

Finger And Toe Cold Gel Ice Pack, By Penguin Fingers ...

Gout causes attacks of painful inflammation in one or more joints. It is a type of arthritis . The pain of a gout attack can be severe.

Gout is caused by a chemical in the blood, called uric acid . Uric acid is usually harmless and is made in the body. Most is passed out with the urine and some from the gut with the stools . In people with gout the amount of uric acid in the blood builds up. From time to time the level may become too high and tiny grit-like crystals of uric acid may form. The crystals typically collect in a joint. The crystals irritate the tissues in the joint to cause inflammation, swelling and pain – a gout attack.

Note: some people have a high level of uric acid but do not form crystals or have gout. Also, rarely, some people with a normal level of uric acid have gout attacks. However, as a rule, the higher the level of uric acid, the greater the chance of developing gout.

Home Remedies And Lifestyle

Gout symptoms are caused by the excessive accumulation of uric acid, a condition known as hyperuricemia. Over time, the build-up can lead to the formation of uric acid crystals in and around a joint, triggering severe and protracted bouts of pain and inflammation.

As such, gout treatment is focused on two things: the reduction of uric acid and the alleviation of gout pain.

How Many Times To Ice And Other Useful Tidbits

Generally, icing in any combination from above methods, is recommended 3-5 times a day. You may ice more than 3-5 times a day. Follow guidelines for each method to avoid cold damage.

For nerve compression syndromes like carpal tunnel, cubital tunnel and radial tunnel, ice pack is recommended for 12 to 15 minutes. For tendonitis, such as tennis elbow or tendonitis of the wrist or thumb, I recommend ice cup direct ice massage at least 1-2 times a day, and ice pack 2-3 times a day to make up the difference, for a total of 3-5 times per day. If you can do the ice massage 3 to 5 times a day, great! Although my specialty as an occupational therapist is upper extremity, I have had ice prescribed to me personally for my knees for arthritis as well as occasional patellar tendonitis. I sometimes use ice packs, but tend to cut to the chase and do direct ice massage for my knees.

The nerves are very close to the surface of the skin at the cubital tunnel, or funny bone on the inside of the elbow, and snuffbox, at the base of the thumb at the wrist. Sometimes, such as when the barometer drops, these areas can be sensitive to direct ice massage. You will experience an intense nerve ache. Try ice pack instead. You can try an extra layer of towel with your ice pack if needed. Another option is a Karo Syrup ice pack, which is not as intensely cold as the slushy ice pack.

As gout frequently develops in the feet, compression socks can be worn over the affected area to boost circulation and remove the build up of urate crystals. It is best to consult with a doctor beforehand to understand whether compression socks would be necessary.

Compression socks come in various pressure levels, which are measured in millimeters of mercury . The greater the level, the stronger the pressure applied. You can refer to the table below to understand the different levels available:

Finger and Toe Cold Gel Ice Pack, by Penguin Fingers ...

You have already understood that ice packs and cold water are the safest options for reducing gout pain. However, you may also soak the affected part in hot water. Make sure that your inflammation is not intense while using the hot water.

Researchers have claimed that hot water can dissolve the urate crystals. Thus, with the regular use of hot water, you can reduce the urate concentration. There will be no formation of crystals as you continue the treatment.

Some patients have found results by using both hot and cold compression techniques. They have applied the hot compress for 3 minutes and switched to a cold compress for around 30 seconds. By doing it repeatedly, you will get positive results after a few days.

Five Ways To Ease A Gout Attack

If you have gout, you know what its like to suffer from a flare-up. Also known as gouty arthritis, a gout attack begins with a burning or itching feeling in your joints. The next sign is redness, swelling, or severe pain in the joint. This is caused by a buildup of uric acid around the joints, which forms uncomfortable uric crystals. This condition can be so painful that anything touching the jointeven a bedsheetcan cause severe pain.

If you are suffering from a gout attack or flare-up, you dont need to agonize alone or in silence. There are a few things you can do to ease the pain and prevent future flare-ups.

    Over-the-counter treatments.

Several over-the-counter treatments can help relieve the pain caused by a gout attack. These nonsteroidal anti-inflammatory drugs can lower uric acid levels and treat episodes of inflammation. Some good choices include Aleve, Motrin, and Advil, and, when taken in prescription doses, can be very effective.

Contact Carolina Arthritis for guidance and to learn more about this strategy for your gout attacks.

    Change your diet.

Many dietary and lifestyle changes can assist in recovery. Foods high in purines can increase the uric acid in your blood and trigger a gout flare-up. Some of these foods include:

    • Some seafood
    • Organ meats like liver
    • Fatty foods

    Ice, elevate, and relax.

    Steroids treatments.

How To Treat A Gout Flare

Different medications can be used to address gout, but when youre in the midst of a gout flare, the number-one goal is pain management. Because the pain is so severe, its really important to try to bring it under control quickly, says Dr. Saag.

Even though gout is caused by uric acid buildup, now is not the time to start a drug thats specifically designed to lower uric acid levels.

Instead, your doctor will likely prescribe a higher dose of a non-steroidal anti-inflammatory drug , such as naproxen, or a steroid medication like prednisone. Your doctor might also inject a steroid directly into the affected joint, or, if the pain is bad enough, start you on an intravenous version of it, says Dr. Saag.

Colchicin is another medication that is sometimes used to manage gout flares. This drug is not a traditional pain reliever, since it doesnt seem to relieve pain caused by other conditions. According to a Cochrane Review, about 40 out of 100 people who use colchine during a gout attack rated their pain 50 percent lower than those who took a placebo, but theres also a high risk of side effects like vomiting, nausea, and diarrhea, especially when taken at high doses.

If youve had gout flares before and are now seeing a new provider or are being treated in the emergency room, be sure to tell the doctor about the treatments youve previously tried during an acute episode and how well they worked for you.

Why Does Uric Acid Build Up

Normally, there is a fine balance between the amount of uric acid that you make and the amount that you pass out in the urine and stools. This keeps the level of uric acid in the blood in check. However, in most people with gout, their kidneys do not pass out enough uric acid and the blood level may rise. They are said to be under-excreters of uric acid. Their kidneys usually work otherwise normally.

In some people, the build-up of uric acid may due to other factors – for example:

Imagine suddenly waking up in the middle of the night with excruciating pain in your big toe. Youd find it difficult to walk, and even the gentle pressure of a duvet resting on the affected foot would be agonising. It sounds like a nightmare, doesnt it? Sadly, this scary situation is a reality for many people who suffer from a type of arthritis known as gout. Thats why its crucial to understand what causes the problem, as well as how to manage and prevent it.

Excess Uric Acid Leads To Gout Attacks

Gout is not only unpredictable and debilitating but also relatively common. According to the UK Gout Society, one in 14 men and one in 35 women will develop this joint disease. The problem occurs mainly in the lower extremities the knees, ankles and feet with big toe joints at the greatest risk. The NHS website includes a the skin looks extremely red and swollen.

People develop gout as a result of hyperuricemia, i.e. high levels of uric acid in the bloodstream. Uric acid crystals build up around the joint at the base of the big toe and other joints, ultimately tricking your body into thinking its under attack. White blood cells try to defend you by surrounding the crystals, leading to the severe joint pain and inflammation familiar to gout sufferers.

Gout pain can last for more than a week and often returns. To reduce the likelihood of permanent joint damage, urgent medical treatment is vital. Your doctor may prescribe medicine to lower uric acid levels, as well as anti-inflammatories.
