What Medications Can Be Used To Lower Uric Acid Levels In The Blood

You will need take daily medicines such as allopurinol or febuxostat . Lowering uric acid to less than 6 mg/dL is needed to prevent deposits of uric acid. If you have visible tophi, the uric acid should be lower than 5 mg/dL. Like many other medications such as for high blood pressure or high cholesterol, urate-lowering medicines are meant to be taken daily for life.

As noted above, starting allopurinol or febuxostat can trigger a flare of gout. This can be prevented by slowly increasing the dose of these medicines, and by taking a low daily dose of colchicine. Talk to your provider about how to prevent a flare, and be aware that if this happens, it is not an allergic reaction.

Though only 1 in 1,000 patient cases, there is a more severe allergic reaction to allopurinol, with fever or rash that can cover the body and deterioration of liver and kidney function. At-risk patient people can be screened for a genetic marker that can help predict this reaction. To minimize this hypersensitivity, providers will start you on a low dose with a step-up plan to the proper medication dose.

In summary, when you start a uric acid-lowering drug, your provider should slowly raise the dose and keep checking your blood uric acid levels regularly. Once your uric acid level falls below a healthy 6.0 mg/dL, crystals tend to dissolve, and new deposits of crystals can be prevented. Also, you will have fewer attacks of gout.

Allopurinol & Tophi Guideline

The first strong recommendation for gout sufferers with tophi is to initiate ULT. That is Urate Lowering Therapy. Which I usually refer to as uric acid treatment. Another strong recommendation is that allopurinol is the first choice of uric acid treatment. But recommended allopurinol dosage is something that you and your doctor need to determine between you. Because your medical history and preferences will determine how quickly you allow allopurinol to dissolve your tophi. There is a lot of detail in the gout guideline to help you and your advisers. In a nutshell… Strong recommendations included initiation of ULT for all patients with tophaceous gout The Voting Panel strongly recommended allopurinol as the preferred first-line agent given its efficacy when dosed appropriately There is ample evidence that lower SU levels hasten the resolution of tophi

How Apple Cider Vinegar Works For Gout

ACV contains antiviral, antibacterial, and antifungal properties. It is full of nutrients, like potassium, amino acids, calcium, and iron, which can help reduce toxin buildup and help rid the body of excess levels of uric acid.

ACV also contains acetic acid, an ingredient that is effective at preventing and treating gout. According to some researchers, acetic acid turns alkaline inside the body and forms a pH-balanced environment that prevents gout.

ACV also contains malic acid. This ingredient is able to dissolve the uric acid in your blood, which could reduce the intense pain and soreness associated with gout.

Drinking apple cider vinegar breaks up the uric acid crystals and thwarts them from reforming in the joints by supporting proper blood circulation. It also promotes joint flexibility by reducing swelling and inflammation in the joints.

Some people even claim to experience gout relief a few hours after drinking apple cider vinegar.

What Are The Surgical Options

Self surgery on Gouty Tophus

There are several surgical options for gout tophi that you may discuss with your healthcare provider:

  • Shaver technique: Intra-lesion shaving is done by making multiple small incisions of the skin to allow direct access to the tophic mass. Suction and irrigation are performed simultaneously to remove the chalky deposits of sodium urate at the same time.
  • Full surgical removal: The tophi may be fully excised and removed from the joint as much as possible without destroying the surrounding tissue. Removal of gouty deposits and nodules occur at this time as well.
  • Joint fusion: If the joint is unstable and damaged by the tophi it may be recommended to have joint fusion. Smaller joints will be fused together to limit movement and reduce pain.
  • Joint replacement: If the gout tophi cause irreversible damage to the joint, you may need a total joint replacement such as in the knee or hip to restore function and reduce pain.

How Will Gout Affect Me

Attacks can vary from person to person. Some people only have an attack every few years, while others have attacks every few months.

Without medication attacks tend to happen more often and other joints can become affected.

Having high urate levels and gout for a long time can lead to other health problems, including:

  • narrowing of the arteries – which can lead to an increased risk of stroke or heart attacks or other heart problems
  • osteoarthritis, which occurs when the urate crystals and hard tophi cause joint damage.
  • an increased risk of developing kidney disease or worsening of the condition if you already have it
  • kidney stones
  • an increased risk of some cancers, especially prostate cancer
  • mental health problems, including depression
  • underactive thyroid
  • erectile dysfunction in men.

If you take medication to lower your urate levels, and have a healthy diet and lifestyle, most of the damage and complications caused by gout can be stopped.

Treatments for gout are incredibly successful. There are two main parts to treating gout, which are:

  • treating the acute attack
  • treatments to prevent future attacks.

Reducing Tophi In Gout

If you’re like Bert Middleton, you have said to yourself, Enough is enough! There must be something I can do to eliminate this gout! I don’t want to go through surgery to eliminate the tophi crystals and I want a permanent solution. I won’t stop until I find something!

Bert discovered that gout was the result of an overly acidic internal environment and that he needed alkalinizing green drinks or alkaline water to restore the alkalinity to his body so that the uric acid levels could be lowered. He also learned that the recommended diet for gout is one that is a low purine diet. This means you can’t eat red meats, sardines and other high purine foods. You have to avoid alcohol too if you want gout to go bye-bye once and for all. Bert lived through it all and ended up with three excruciatingly painful surgeries for gout tophi before he found the answer.

How Are Gout Crystals Removed

If youve been diagnosed with gout, you may wonder, can gout be removed? Gout can be an extremely painful condition. A commonf inflammatory arthritis that affects the joints, most often the big toe, gout is caused by a buildup of uric acid in the body.

With proper treatment you may be able to reduce the number of gout crystals in your joints, however, sometimes gout tophi can interfere with your daily life. Gout removal may then become necessary. When this occurs, you may need treatment such as steroids, debridement, or full surgical removal in order to relieve pain, avoid infection, or restore mobility.

Baseline Characteristics Of Gout Patients With And Without Tophi

Gout Busting Out!: Gout Crystals Breaking Though The Skin Of The Big Toe

From a total of 257 patients with gout, 66 had tophi whereas the remaining 191 had no tophi at the time of the initial visit. The baseline characteristics of the study patients with and without tophi are shown in Table . The mean age of patients in the tophi group at diagnosis was significantly higher than that of patients in the no tophi group . The gender distribution was similar between the 2 groups. The median duration of symptoms prior to the diagnosis of gout was significantly longer in the tophi group than in the no tophi group . The 2 groups did not differ with respect to factors such as a history of renal stones, the family history, number of gout flares , concomitant medication , type of urate lowering therapy , presence of comorbidities . Among the study subjects, 162 patients were initially treated with febuxostat and 95 patients were treated with allopurinol. In the febuxostat group, 160/162 patients continued taking febuxostat during the follow-up period, and 2 patients discontinued the drug. In the allopurinol group, 84 patients changed to febuxostat and 11 patients stopped taking allopurinol. At baseline, the uric acid, blood urea nitrogen , creatinine and estimated glomerular filtration rate values, too, did not differ between the 2 groups.

Krystexxa Can Resolve Tophi That Form When Uric Acid Crystals Build Up Under Your Skin

Tophi are telltale signs your gout is out of control. When uric acid levels remain high, deposits of uric acid crystals can develop into hard visible lumps that damage your joints and bones. Tophi can be microscopic or large and, though rare, may need to be surgically removed. Its also possible for tophi to erupt on their own.

Who Is Most Likely To Get Gout Tophi

Someone who has been diagnosed with gout but rarely has a flare up perhaps you have an attack once or twice a year will be less likely to develop tophi.

Youre most apt to get these nodules if you have frequent, severe attacks. Frequent episodes of gout are an indication that your uric acid levels are consistently too high. That can happen to anyone with gout, but its more likely if you dont follow your doctors instructions and dont take your prescribed medication to keep uric acid levels in check.

I had tophi in my spine years ago, and said it was just incidental. Now having another gout attack in my spine , and its all coming together. Severe, severe pain. I am serious this time about getting it together. Last gout attack I had when I was on the Keto diet , and I followed it strictly. Back to sane eating without alcohol and red meat.

This tophi is new to me. I thought Id developed a bunch of warts on my fingers. I asked my Doc. What to buy to get rid of the warts and she told me my gout had developed into these things. So, Im just starting working with the tophi and hope I can clear them up.

Im a chronic gout suffer and have had attacks in my toes, feet, knees, and elbows.Currently, I have what I think is tophi in both my elbows but it doesnt look anything like the image in the article. Is tophi solid or is soft and pliable or can it be both? I have zero pain but I would like to remove it for appearance reasons.

  • June 12, 2018

    Hi Jason!

It can be both solid or soft and pliable as you say. Go visit couple of rheumatologists and see what advice they each give you on how to get rid of it and take it from there.

    Good luck!

    What Are Gout Tophi

Gout tophi are masses of monosodium urate crystals that have deposited around a joint. These tophi usually occur over time after several gout attacks. In the hands, they look like bulbous lumps that have attached themselves to the joints.

You may not experience much pain or disability at first, but the tophi, if left untreated, can continue to grow to the point that it is difficult to carry out daily functions. Untreated tophi and gout can cause joint instability and limit your range of motion. Tophi can also cause bone erosions and complications such as ulcerations and infections.

    When Is Surgery Considered

If you experience a lack of improvement after more conservative treatments, surgery may be necessary. There are multiple instances when surgery for gout tophi is considered.

You may need surgery for impaired function, nerve compression, infection, joint instability, significant pain, and skin ulceration. Ulcerated gouty tophi are susceptible to infections. Surgery is considered when or infection occurs.

If there is deformation or inability to move the joint properly, surgical intervention becomes inevitable. It is most common to have surgery when mechanical problems result from the tophi. Sometimes, surgery may be indicated for pain.

What Can Trigger A Gout Attack

Several things can cause the crystals to shake loose into your joint cavity, triggering an attack. These include:

    • a knock or injury to the joint
    • an illness that may make you feverish
    • having an operation
    • having an unusually large meal, especially a fatty meal
    • drinking too much alcohol
    • dehydration
    • starting urate lowering therapy, especially at a high dose, or not taking your treatment regularly each day.

    Target Sua In Chronic Gout

Since it is clear that gout is the consequence of the accumulation of uric acid in the body, the logical way to treat the condition is to lower the sUA level and deplete the body urate pool. The clinical manifestations of gout are due to deposition of MSU crystals and if the crystals are dissolved completely and no new crystals can form, then the condition is cured. To achieve this, the sUA must be reduced below the saturation point of MSU under physiological conditions.

This has been recognized in recent evidence-based recommendations from the European League against Rheumatism Task Force for Gout, which recommend that the sUA should be reduced to a target of 6 mg/dl . The authors of the recommendations point out that the target sUA level should be linked to the saturation level of MSU rather than to the normal laboratory range, which can vary between populations and with time. It is also acknowledged in the guidelines that the target sUA may vary depending on the characteristics of the patient and a lower target may be appropriate in patients with extensive crystal deposition. The British Society of Rheumatology has also published guidelines for the management of gout and these recommend a stricter sUA target of < 5 mg/dl .

    Diagnosis And Management Of Gout

JOEL R. PITTMAN, PHARM. D., and MICHAEL H. BROSS, M.D., University of Mississippi Medical Center, Jackson, Mississippi

    Am Fam Physician. 1999 Apr 1 59:1799-1806.

Gout is a disease resulting from the deposition of urate crystals caused by the overproduction or underexcretion of uric acid. The disease is often, but not always, associated with elevated serum uric acid levels. Clinical manifestations include acute and chronic arthritis, tophi, interstitial renal disease and uric acid nephrolithiasis. The diagnosis is based on the identification of uric acid crystals in joints, tissues or body fluids. Treatment goals include termination of the acute attack, prevention of recurrent attacks and prevention of complications associated with the deposition of urate crystals in tissues. Pharmacologic management remains the mainstay of treatment. Acute attacks may be terminated with the use of nonsteroidal anti-inflammatory agents, colchicine or intra-articular injections of corticosteroids. Probenecid, sulfinpyrazone and allopurinol can be used to prevent recurrent attacks. Obesity, alcohol intake and certain foods and medications can contribute to hyperuricemia. These potentially exacerbating factors should be identified and modified.

    Are Tophi Painful

Tophi are not usually painful, but there are exceptions. They can become infected, which is likely to be painful. Its also possible for tophi to press on a nerve, which can be both painful and potentially dangerous.

The biggest common problem, aside from the aesthetics of having tophi the appearance can be distressing for some patients, says Dr. Quinn is that tophi can damage joints, destroy cartilage, and lead to bone erosion. Over time, this can lead to significant deformity and disability from gout, as it becomes harder to move the affected joints.
