You will need take daily medicines such as allopurinol or febuxostat . Lowering uric acid to less than 6 mg/dL is needed to prevent deposits of uric acid. If you have visible tophi, the uric acid should be lower than 5 mg/dL. Like many other medications such as for high blood pressure or high cholesterol, urate-lowering medicines are meant to be taken daily for life.

As noted above, starting allopurinol or febuxostat can trigger a flare of gout. This can be prevented by slowly increasing the dose of these medicines, and by taking a low daily dose of colchicine. Among the study subjects, 162 patients were initially treated with febuxostat and 95 patients were treated with allopurinol. In the febuxostat group, 160/162 patients continued taking febuxostat during the follow-up period, and 2 patients discontinued the drug. In the allopurinol group, 84 patients changed to febuxostat and 11 patients stopped taking allopurinol. At baseline, the uric acid, blood urea nitrogen , creatinine and estimated glomerular filtration rate values, too, did not differ between the 2 groups.

    I had tophi in my spine years ago, and said it was just incidental. Now having another gout attack in my spine , and its all coming together. Severe, severe pain. I am serious this time about getting it together. Last gout attack I had when I was on the Keto diet , and I followed it strictly. Back to sane eating without alcohol and red meat.

    Im a chronic gout suffer and have had attacks in my toes, feet, knees, and elbows.Currently, I have what I think is tophi in both my elbows but it doesnt look anything like the image in the article. Is tophi solid or is soft and pliable or can it be both? I have zero pain but I would like to remove it for appearance reasons.

    What Are Gout Tophi

    Gout tophi are masses of monosodium urate crystals that have deposited around a joint. These tophi usually occur over time after several gout attacks. In the hands, they look like bulbous lumps that have attached themselves to the joints.

    You may not experience much pain or disability at first, but the tophi, if left untreated, can continue to grow to the point that it is difficult to carry out daily functions. Untreated tophi and gout can cause joint instability and limit your range of motion. Tophi can also cause bone erosions and complications such as ulcerations and infections.

    When Is Surgery Considered

    If you experience a lack of improvement after more conservative treatments, surgery may be necessary. There are multiple instances when surgery for gout tophi is considered.

    Gout is a disease resulting from the deposition of urate crystals caused by the overproduction or underexcretion of uric acid. The disease is often, but not always, associated with elevated serum uric acid levels. Clinical manifestations include acute and chronic arthritis, tophi, interstitial renal disease and uric acid nephrolithiasis. The diagnosis is based on the identification of uric acid crystals in joints, tissues or body fluids. Treatment goals include termination of the acute attack, prevention of recurrent attacks and prevention of complications associated with the deposition of urate crystals in tissues. Pharmacologic management remains the mainstay of treatment. Acute attacks may be terminated with the use of nonsteroidal anti-inflammatory agents, colchicine or intra-articular injections of corticosteroids. Probenecid, sulfinpyrazone and allopurinol can be used to prevent recurrent attacks. Obesity, alcohol intake and certain foods and medications can contribute to hyperuricemia. These potentially exacerbating factors should be identified and modified.
