How To Know If Your Gout Is Progressing
As you become more familiar with gout symptoms, you may be able to sense that a gout attack is coming on. Worsening of pain, swelling, redness, and warmth of the affected joint during the attack is the sign of progression of that attack, Dr. Meysami says.
In addition, the disease overall may progress with recurrent or more frequent gout attacks with longer duration, the involvement of more joints, and the presence of tophi, Dr. Meysami says.
If you have more than one gout flare a year, its really important to get on a regular gout medication, says Dr. Fields.

How Can An Attack Of Gout Be Treated
The management of an acute attack of gout is very different from the prevention of subsequent attacks.
Treatments used for prevention, such as allopurinol can actually make things worse if given during an attack, and so need to be held back until the attack has resolved for several weeks.
There are a number of measures that can help resolve an attack of gout. See Table 2 for summary of treatment strategies for acute gout. One principle is that treatment for an attack of gout should be instituted quickly, since quick treatment can often be rewarded with a quick improvement.
If an attack of gout is allowed to last more than a day or so before treatment is started, the response to treatment may be much slower.

What Are The Symptoms Of Gout
An episode of gout is called a gout attack. Gout attacks are very painful and can happen quite suddenly, often overnight. During a gout attack, symptoms in the affected joint may include:
- Intense pain.
- Tenderness, even to light touch, such as from a bedsheet.
- Warmth, or a feeling like the joint is on fire.

Arthritis / Acute Gout Attack
Gout is a form of arthritis, hence it causes pain and discomfort in the joints. A typical gout attack is characterized by the sudden onset of severe pain, swelling, warmth, and redness of a joint. The clinical presentation of acute gouty arthritis is not subtle with very few mimics other than a bacterial infection.
The joint most commonly involved in gout is the first metatarsophalangeal joint , and is called podagra. Any joint may be involved in a gout attack with the most frequent sites being in the feet, ankles, knees, and elbows.
An acute gout attack will generally reach its peak 12-24 hours after onset, and then will slowly begin to resolve even without treatment. Full recovery from a gout attack takes approximately 7-14 days.
An accurate and colorful discription of a gout attack was elegantly written in 1683 by Dr. Thomas Sydenham who was himself a sufferer of gout:

I found that adding Celery to my diet has greatly reduced my uric acid levels. I also take other supplements but definitely Celery has had the biggest impact on controlling my gout.

Signs And Symptoms Of Gout
Any joint can be affected by gout, but it usually affects joints towards the ends of the limbs, such as the toes, ankles, knees and fingers.
Signs and symptoms of gout include:
- severe pain in one or more joints
- the joint feeling hot and very tender
- swelling in and around the affected joint
- red, shiny skin over the affected joint
Symptoms develop rapidly over a few hours and typically last three to 10 days. After this time the pain should pass and the joint should return to normal.
Almost everyone with gout will experience further attacks at some point, usually within a year.

How Long Does Gout Last
Discover here how long your gout attack can last for and who is at risk of longer gout attacks. Then discover how to prevent them.
Generally speaking, an attack of gout can last anywhere from 3 to 21 days, sometimes even longer. 7 to 10 days could be the average for most people.
But a lot depends on how quickly it is diagnosed, how quickly treatment is started, the type of treatment used, and its location in the body.
But there are also other determinants for how long a gout attack can last. For example, gout may last longer in:
- folks with certain underlying medical conditions
- and even some medications
The big toe is the most common location for gout, but you can also get gout in the knee, the ankle, hand, and so on. Basically any location where uric acid crystals can accumulate
You see, the common denominator in gout is high uric acid blood levels a condition called hyperuricemia. This condition can then lead to the formation of uric acid crystals in the joints. Its generally assumed that high uric acid is anything over 6 mg/dl. However, it has been known for some people to have levels over 9.0 mg/dl and not suffer from gout. Equally, gout has been known in patients with levels as low as 6 mg/dl. Much depends on the individual. But, as a general rule of thumb, maintaining your levels below 6 mg/dl is a good target.

Tips To Deal With An Attack And Prevent Future Ones
Gout is a condition characterized by flare-ups of joint pain and inflammation brought on by the crystallization of excessive levels of uric acid in the affected joint. Other symptoms of gout resemble those of flu, such as fever and muscle aches
There is not cure for gout and coping with the condition can be challenging: Flare-ups can occur with no warning and last days or weeks, significantly affecting quality of life. However, gout can be effectively managed with a combination of lifestyle measures and medication.
Get Tested For Sleep Apnea
Untreated sleep apnea can increase the risk of gout.12,13 Sleep apnea is a sleep disorder that causes the body to take in less oxygen while sleeping, which in turn causes the body to produce more purines.12,14 Sleep apnea is typically treated with a continuous positive airway pressure machine or other device that increases oxygen intake while sleeping.

The Role Of Physical Activity In Prevention Of Gout
Along with diet, physical activity can help with weight loss, and gout has been associated with being overweight.7 in patients with well-established gout, especially if X-rays have demonstrated joint damage in the foot, a low-impact exercise program is reasonable. An exercise program combined with diet in gout can reduce risk for attacks.7 If an attack seems to be coming on in the lower extremity, patients are well-advised to try to get off their feet, since impact seems to worsen gout attacks. Clues to an attack of gout coming on include local swelling, heat, redness, and tenderness in a joint, especially in the foot, ankle, or knee. Some patients have fever and chills as the first warning that an attack of gout is coming on.

Lessen Your Risk Of Early Death
On average, having gout raises a persons risk of dying earlier than a similar person without gout by 25 percent. To lessen that risk, gout patients need to work with their doctor to lower uric acid levels to the target of < 6 mg/dl and keep it there throughout life, Dr. Edwards says. In addition, people with gout need to control medical conditions that go along with gout , such as hypertension, heart disease and kidney disease.
So if you have gout, heres what you need to do to ensure you live the longest, healthiest life you can:
- Take your gout medications and other medications as directed
- Get screened and treated for comorbid conditions

What Are Future Possible Treatments Of Gout
Fortunately, present medications are successful in the vast majority of gout patients. But some patients cannot tolerate our present arsenal of gout medications. For others, these agents are not sufficiently effective. Therefore, new treatments are continually being sought. Some of the more promising include anakinra, rilonacept, canakinumab, BCX4208 and arhalofenate.
Home Care For A Gout Flare
If your doctor has diagnosed you with gout and given you medicine for a flare-up, take the medicine as directed when you know youâre having one. In most cases, that will probably be as soon as the first signs begin.
Your doctor may prescribe nonsteroidal anti-inflammatory drugs such as celecoxib, indomethacin, meloxicam, or sulindac or suggest you take over-the-counter NSAIDs, like naproxen or ibuprofen. Depending on your medical history, your doctor may prescribe steroids or other medicines to reduce inflammation, such as colchicine .
In some cases, you already may be taking medicine like colchicine to prevent gout flare-ups. Your doctor may have also suggested:
If you canât take allopurinol or it is not effective, your doctor may prescribe . It should be used with caution, however, because it has been linked to increased risk of death from heart disease and from other causes.
Just because you have a flare doesn’t mean these medicines aren’t working. In the first few months that you take them, you may have an attack as your body adjusts to the drug. Your doctor will likely have given you something to take if this happens, too.
If youâve been taking preventive gout medicine for a long time and youâre having flares for the first time in a while, call your doctor. They may talk to you about changing your dosage or your medicine.

Ascertainment Of Gout Flares
All members from the cohort of incident gout were followed from start date, corresponding to 30 days after the first-ever diagnosis of gout, until the earliest occurrence of one of the following endpoints: gout flare, cancer, death, end of practice data collection or 31 December 2008, whichever came first.
The operational definition of gout flare was defined as follows: when there was a recorded prescription of colchicine or when there was a health-care visit recording gout together with at least one of the following treatment patterns within 1 week: IA aspiration, IA injection , prescription of NSAIDs or prescription of CS or adrenocorticotropic hormone . The final cohort consisted of 23857 patients: the difference with the initial cohort is patients presenting one of the censoring criteria during the first 30 days after the diagnosis of gout .
Ascertainment of flares was performed recursively adding in every new follow-up a period of grace of 30 days to the date of the flare detected in each consecutive search . The validity of our operational definition of flare was confirmed based on the manual review of a random sample of 100 computerized patient profiles .

Can It Lead To Any Complications
If left unmanaged, gout-related inflammation can cause permanent damage to your ankle joint, especially if you have frequent flare-ups.
Over time, lumps of uric acid crystals, called tophi, can also form around your ankle. These lumps arent painful, but they can cause additional swelling and tenderness during a flare-up.
