When Should Nsaids Be Used In The Treatment Of Gout

NSAIDs are the drugs of choice in most patients with acute gout who do not have underlying health problems. Although indomethacin is the NSAID traditionally chosen for acute gout, most of the other NSAIDs can be used as well. Select an agent with a quick onset of action. Do not use aspirin, because it can alter uric acid levels and potentially prolong and intensify an acute attack. Low-dose aspirin alters uric acid levels, increasing the risk of gout attacks and requiring close uric acid monitoring when aspirin is added to a uric acid/gout treatment regimen.

Cyclooxygenase-2 inhibitors have been used with success, but patients may require higher dosages than are typically used.

Avoid NSAIDs in patients with a history of peptic ulcer disease or gastrointestinal bleeding, those with renal insufficiency or abnormal hepatic function, those taking warfarin , and those in the intensive care unit who are predisposed to gastritis. Limit NSAID use in elderly patients, because of the potential for adverse central nervous system effects. Use NSAIDs cautiously in patients with diabetes and those who are receiving concomitant angiotensin-converting enzyme inhibitors.

Side Effects Of Indomethacin And Naproxen

As stated earlier, as an NSAID, both of the medications have a fair share of the side effects, mainly gastrointestinal bleeding and peptic ulcer, including the fatal cardiovascular effects. So if youre okay with the bleeding inside out just to take away the pain temporarily, go on and take one of them. The Naproxen is somewhat better than the indomethacin due to the latter, is known to cause more severe cases of the side effects.

Both of these medications, together with some other medications out there, just masks the symptoms. If youre going to plan in solving the condition once and for all, you must think out of the box, open your mind, and get rid of the uric acid.

    Common Questions About Naproxen

Naproxen is a non-steroidal anti-inflammatory drug . It works by reducing hormones that cause inflammation and pain in the body.

Paracetamol is usually the best treatment for most types of pain, but naproxen is better for some types, such as period pain or back pain.

You should start to feel better 1 hour after taking naproxen.

But it might take up to 3 days for naproxen to work properly if you take it regularly twice a day.

Depending on why you’re taking naproxen, you may only need to take it for a short time.

For example, if you have a sore back or period pain, you may only need to take naproxen for 1 or 2 days.

You may need to take it for longer if you have a long-term condition, such as rheumatoid arthritis.

If you need to take naproxen for a long time, your doctor may prescribe a medicine to protect your stomach from side effects.

It’s best to take the lowest dose of naproxen for the shortest time to control your symptoms.

Talk to your doctor if you’re unsure how long you need to take naproxen for.

Naproxen can cause an ulcer in your stomach or gut if you take it for a long time or in big doses.

There’s also a small risk that people taking very big doses for a long time may get heart failure or kidney failure.

It’s best to take the lowest dose that works for the shortest possible time.

If you need to take naproxen very often or you’re taking a big dose, talk to your doctor about your pain.

Naproxen does not work for some types of pain, such as nerve pain.

    Naproxen Recommended Over Low

medwireNews: Naproxen and low-dose colchicine are similarly effective in the reduction of pain associated with gout flares, but patients receiving colchicine tend to experience more side-effects, show results from the multicenter, open-label CONTACT trial.

In the absence of contraindications, naproxen should be used ahead of low-dose colchicine in primary care on the grounds of effectiveness, safety and cost, Edward Roddy and colleagues write in the Annals of the Rheumatic Diseases.

The researchers found that among the 200 patients with a gout flare who were randomly assigned to received naproxen 750 mg immediately then 250 mg every 8 hours for 7 days, the mean pain score fell from 6.7 points on day 1 to 1.4 points on day 7.

After adjustment for age, sex, and baseline pain score, there was no significant difference between the two groups in the change in worst pain intensity averaged over days 17, at 3.8 versus 3.5 points for naproxen and colchicine, respectively.

There were also no differences between the two groups at any time point in the level of complete pain resolution or patient global assessment of treatment response.

Furthermore, at week 4, the proportion of patients reporting a relapse/recurrent gout flare, consulting a primary care provider, practice nurse, or emergency department, or needing time off work was similar between the groups.

Is Naproxen a Blood Thinner: Pain Medication That Is At Risk

It is important to note that whenever starting a uric acid lowering treatment, there is a risk of precipitating a gout flare. A plan should be in place for management if this occurs. This generally can be avoided with the co-administration of prophylactic medications along with the uric acid lowering therapy.

    Probenecid

Probenecid may be given to patients with decreased clearance of uric acid by the kidney and normal renal function. In general its use should be limited to patients under the age of 60. Probenecid acts by inhibiting reabsorption of uric acid in the proximal tubules of the kidney. Starting dose is at 500 mg to 1000 mg daily and increased to 1500 mg to 2000 mg as needed. Occasionally higher doses are needed. Probenecid may precipitate renal stone formation and good oral hydration should be encouraged. Probenecid is contraindicated in patients with renal stones and in patients with urate nephropathy. Probenecid given inappropriately to patients with hyperuricemia due to overproduction of uric acid can cause renal stones and urate nephropathy.

Naproxen Caused Fewer Side Effects Supporting Naproxen As First

When youre in the midst of a painful gout flare, you want it to stop fast. But theres more than one to treat a flare, so doctors have to weigh the pros and cons of the various options. Those include non-steroidal anti-inflammatory drugs like naproxen, which reduce inflammation, as well as a medication called colchicine. The exact mechanism of action for colchicine is unknown, but it may interfere with the bodys immune response that kicks in when uric acid crystallizes in a joint.

All drugs have side effects. High doses of colchicine tend to be effective, but they can cause severe GI side effects. Lower doses of colchicine can also be used, but it hasnt been clear whether cutting the dose really preserves the benefit while reducing side effects.

Meanwhile, naproxen has the potential to cause some stomach upset or raise your blood pressure.

So should your doctor choose naproxen or low-dose colchicine? A new study the first one to directly compare these treatment options comes out in favor of naproxen.

In this study, which appears in the journal Annals of the Rheumatic Diseases, UK researchers conducted a multicenter open-label trial in which nearly 400 adults who showed up at a primary care doctors office with a gout flare were randomly assigned to get either low-dose colchicine or an initial 750 mg dose of naproxen followed by an additional 250 mg of naproxen every eight hours for a week.

In other words, naproxen provided the same benefit with fewer risks.
