Gout is a type of arthritis caused by a buildup of uric acid and characterized by foot pain, swelling, redness, and difficulty walking is typically localized in the big toe. However, some types of gout may be localized in the heel, making it difficult to distinguish from plantar fasciitis.

Lets explore the connection between gout and heel pain, as well as symptoms that can help you tell the difference between plantar fasciitis and gout

What Are Heel Spurs

Believe it or not, and I can’t stress this enough, heel spurs don’t actually cause pain! Most heel pain is caused by a condition known as plantar fasciitis. When the plantar fascia becomes chronically inflamed, it can get calcified where it is attached to the heel bone. This appears on an x-ray as a heel spur! But rest assured, this spur is likely not causing your pain. The pain is likely from plantar fasciitis, or a chronic strain of the ligament on the bottom of the foot, right where it attaches to the heel. From simple physical activities, such as walking and standing, to more strenuous weight-bearing exercises, such as dancing and jogging, the feet are constantly subjected to significant stresses. Due to it’s strategic location and function, this ligament is vulnerable to damage, injury and pain. One of the most common causes of heel and arch pain is overuse/ repetitive exertion with inadequately supportive shoes.

How come gout hurts, where does all that pain come from? Simple, when the uric acid accumulate in the blood and tissues it can crystallise out into a joint, forming a multitude of tiny, jagged, needle-shaped crystals. This triggers an intense inflammatory response that causes a painful arthritis attack called a gout attack. Have you ever stood on a pin or a needle, perhaps some broken glass and got annoyed and irritated by this? Then you will know how a gout sufferer will feel they can be most irritated and annoyed like a child who has just stood on a pin cushion. Pain has a way of bringing out the worst in many people! As I mentioned, gout commonly strikes the big toe, but other joints can be affected, including the instep, ankle, knee, wrist, elbow and fingers. Acute pain is generally the first symptom and then the affected joint becomes inflamed : red, swollen, hot and extremely sensitive to the touch. I have many times seen a case of gout in the clinic that the patients doctor referred to as osteoarthritis, when in fact it had all the makings of gout. Signs and symptoms such as a needle-like pain affecting the small joint, male patient who enjoys alcohol drinker, crayfish and roe , enjoys barbeques and steaks, etc. Common things happen to people commonly, I have often said.

What Is Gout In Your Heel

Some people with gout, also known as gouty arthritis, say an attack begins with a burning, itching, or tingling feeling in a joint maybe an hour or two before the flare-up starts. The joint may feel a little stiff or a bit sore. Not long after, the telltale signs of gout begin. If you get repeated attacks, you’ll learn your body’s signals that one is about to begin.

Sometimes, people with gout have no early signs that a flare is about to start. They may just wake up in the middle of the night with a very painful joint.

When the flare starts, most people have redness, swelling, and severe pain, usually in one joint. The most common place for gout is the base of the big toe, but it can happen in other joints such as the elbow, knee, wrist, ankle, and instep.

Build Muscle With Strength Exercises

Having strong muscles can protect your joint from wear and tear, especially for joints affected by gout. Beyond weight training, simple resistance exercises can be effective to build muscle. For example, try an elastic resistance band by holding each end and putting your foot in the middle, then repeat your flexibility exercises while pushing against the force of the band.

The key with any exercise after a gout flare is to go slow and listen to your body, Dr. Iversen stresses.

This is because of the uncontrolled formation of uric acid from purine metabolism. The quantity is so excessive that it cannot be flushed out through the kidneys. Middle-aged people and women after menopause are at higher risk of gout. Family history, intake of purine-rich foods, being overweight, alcohol abuse, and an unhealthy lifestyle is also some of the risk factors for gout.

What Are Future Possible Treatments Of Gout

Fortunately, present medications are successful in the vast majority of gout patients. But some patients cannot tolerate our present arsenal of gout medications. For others, these agents are not sufficiently effective. Therefore, new treatments are continually being sought. Some of the more promising include anakinra, rilonacept, canakinumab, BCX4208 and arhalofenate.

Signs Your Pain Is Likely Gout And Not Something Else

If you develop sharp sudden pain in a single joint or a couple of joints, if the pain is so debilitating that it is hard to walk or wear shoes, and if you have risk factors for gout , theres good reason to suspect gout as the culprit. However, its important to see a doctor for a thorough exam and proper diagnosis.

Dr. FitzGerald cautions that issues unrelated to gout can cause an angry, inflamed joint. Gout may be confused with several other conditions that can cause similar symptoms, including:

  • Pseudogout
  • An infected joint
  • Bacterial skin infection
  • Rheumatoid arthritis
  • Psoriatic arthritis

Pseudogout is caused by a different kind of crystal, calcium pyrophosphate. A flare of pseudogout can resemble gout, but it more often affects your wrist and knee, and is unlikely to involve the big toe. Like gout, pseudogout is also considered a form of inflammatory arthritis.     After a gout flare subsides, Dr. Iversen suggests aquatic exercises may be a good way to start re-engaging in exercise because the buoyancy of the water will reduce the impact on the joints.

    Low-impact aerobic exercises can be helpful, too, such as on an elliptical machine. She says its important to keep your joints flexible by incorporating stretching and range-of-motion exercises once the gout flare subsides to promote good joint movement.

    Be careful not to overdo it once you ease into post-flare exercise, she cautions. Patients should not be experiencing pain when walking once the flare has subsided. If you do experience pain with walking after a flare, go back to using a walking support and reduce your planned exercise until the pain subsides.

    If your doctor diagnoses gout, they will most likely suggest medication and certain lifestyle changes based on findings in the testing and your current health.

    Certain medications treat gout attacks or flare-ups. Others reduce the risk of potential gout complications.

    When Is Surgery Considered For Gout

    The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone. Whenever possible, however, we try to avoid surgery to remove tophi. The problem is that the crystals are often extensive, and track back to the bone, so there is not a good healing surface once the tophus is removed. In some rare cases, such as when a tophus is infected or when its location is causing major disability, surgical removal may be considered.

    Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase , since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus.
