One of the only ways allopurinol will be forbidden from therapy in gout is related to renal impairment. Patients with kidney damage can have life-threatening complications when they start using allopurinol, either in the acute or the chronic phase. Renal injury is a risk factor to develop a condition called allopurinol hypersensitivity syndrome.

Allopurinol hypersensitivity syndrome is a severe systemic reaction to allopurinol. It includes symptoms such as fever, skin manifestations, eosinophilia , and multiorgan dysfunction. There is a 25% mortality in patients who develop this syndrome, which is quite high and alarming. Thus, even though there are guidelines to prevent this from happening, doctors might prefer to use other urate-lowering alternatives in this group of patients.

In a nutshell, the main goal of allopurinol is to lower uric acid levels in chronic gout patients. There is current debate on the matter, but the majority of doctors agree that initiating allopurinol in patients recently diagnosed with acute gout is not recommended and does not have any benefit. It is only maintained in patients with chronic gout under treatment with allopurinol, and the only side effect doctors will try to avoid is allopurinol hypersensitivity syndrome, which is more common in patients with renal impairment.

Gout is characterised by the acute onset of excruciating joint pain, with associated swelling, erythema and tenderness. The first metatarsophalangeal joint is most commonly affected in the first attack .

Always consider bacterial infection in the differential diagnosis. Other causes of an acute monoarthritis include other crystal arthropathies, such as pseudogout, osteoarthritis, trauma, and rheumatoid and seronegative arthritis.

Gout can be diagnosed on the basis of good history-taking and examination, but when in doubt, the affected joint should be aspirated.

The underlying risk factor for gout is an elevated serum uric acid level, but not all patients with hyperuricaemia will develop the disease. Hence, the clinician should be wary of checking a uric acid level in a well person with no history of gout. Typical Dosing For Allopurinol

The daily dose of allopurinol will depend on your body’s levels of uric acid. The dose can be taken once a day or in divided doses throughout the day. The maximum daily dose is 800 mg. The typical starting dose for each indication is:

• Gout: 100 mg daily

• High uric acid in cancer: 600-800 mg daily for 2 or 3 days with lots of fluids

• Recurrent kidney stones: 200-300 mg daily

The Difference Between Acute And Chronic Gout

The first step we should take is to differentiate acute and chronic gout. They are different manifestations of the same disease. Gout is not only diagnosed after blood tests with high levels of uric acid. This is called hyperuricemia. The diagnosis of gout requires a physical and functional examination to evidence that the articulations of the patient are being affected by such levels of uric acid in the blood. Otherwise, hyperuricemia is not diagnosed as gout.

Acute gout is characterized by severe pain located in the toes or hands, but not limited to them. The symptoms appear suddenly, sometimes in the middle of the night, and become unbearable and very severe.

The area becomes swollen, red, tender, and very warm because theres a severe inflammatory process triggered by uric acid crystals. Pain in acute gout may last for hours, days, and sometimes a whole week. But after the attack has gone, the patient does not display any symptoms.

What Other Information Should I Know

Keep all appointments with your doctor and the laboratory. Your doctor will order certain lab tests to check your body’s response to allopurinol.

Do not let anyone else take your medication. Ask your pharmacist any questions you have about refilling your prescription.

At the correct dose, all uric acid lowering treatments will cause uric acid crystals to dissolve. Remember, this is what we must do to avoid skin-bursting tophi and crippling joint deposits.

Unfortunately, as they are dissolving, uric acid crystals shed the protein coating, and the immune system starts to attack again, which may result in a painful gout flare. You can take allopurinol with paracetamol and anti-inflammatory medicines such as ibuprofen, naproxen, or indomethacin.

Your doctor may prescribe a NSAID (such as diclofenac or naproxen or a medicine called colchicine to help prevent or to deal with attacks of gout especially in the early stages of allopurinol treatment.

How Can An Attack Of Gout Be Treated

The management of an acute attack of gout is very different from the prevention of subsequent attacks.

Allopurinol is an acclaimed very effective medication in the reduction of uric acid. Coming along with this is febuxostat, these two are the most common of all the medications that help in lowering the uric acid.

In spite of being recommended as a medication for gout, the allopurinol may worsen the gout attacks. The pain may become more severe and more intense. This has something to do with the mechanism of the allopurinol.

The allopurinol, as we know, is a xanthine oxidase inhibitor. Meaning, it prevents the xanthine oxidase enzyme from responding with purine. If the purine doesnt react with the xanthine oxidase, it isnt broken down into uric acid.

Simply speaking, the allopurinol ensures the binding with the xanthine oxidase rather than having purine bind with the enzyme. It just distracts the xanthine oxidase.

Now, to answer the question why does allopurinol worsen acute gout? we need to understand the mechanism of gout attacks first.

Important Things To Remember

How Is It Taken

Allopurinol is taken as a tablet once a day. It is usually better to take it just after eating and the tablet should be swallowed with water. It is important to drink plenty of water during the day because this will help you get rid of more urate through your kidneys.

Your dose of allopurinol may change over time, depending on the amount of urate in your body. Your doctor will monitor your urate levels with blood tests every 2-4 weeks, until they are sure that the dose that youre taking is high enough to reduce the amount of urate in your body.

You may need to remain on a lower dose if you have kidney or liver problems.
