Which Joints Are Involved In Gouty Arthritis And Why Is It Most Common In The Foot

As with all other known types of arthritis, Gout has particular joints it tends to attack, and the foot is its most common location. Gout especially favors the bunion joint, known as the first metatarsophalangeal joint , but the ankle, midfoot and knee are also common locations, as is the bursa that overlies the elbow.

The bunion joint is the first joint involved in 75% of patients and is ultimately involved in over 90% of those with this condition. . It is thought that this joint is especially involved in gout because it is the joint that receives the highest pounds per square inch of pressure when walking or running.

Late in gout, if untreated, multiple joints can be involved, including the fingers and wrists. The shoulder joint is very rarely involved by gout and the same is true of the hip.

When Is Surgery Considered For Gout

The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone. Whenever possible, however, we try to avoid surgery to remove tophi. The problem is that the crystals are often extensive, and track back to the bone, so there is not a good healing surface once the tophus is removed. In some rare cases, such as when a tophus is infected or when its location is causing major disability, surgical removal may be considered.

Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase , since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus.

    Gout Flare

The rich mans disease can be one of the most painful chronic ailments out there. Gout is the unpleasant result of the build-up of uric acid in joints, particularly in the feet. It can be hard to do anything and bring sufferers lives to a standstill. Keeping gout outbreaks under control is no easy task, but there are a few natural options that might alleviate at least some of the pain.

    1. Exercise

One of the worst things about gout is that the worse the symptoms get, the harder it is to exercise. This is a problem because regular exercise is widely regarded as one of the best ways to reduce the symptoms of gout in the long term. Your doctor is the best person to advise you on what exercise is best for you, but generally speaking the best options are low-impact activities. Swimming, yoga and tai-chi are all popular options.

    2. Celery

Celery is often hailed as gouts kryptonite. Along with being an anti-inflammatory, celery is a diuretic, meaning it will make you pee more. This is better than it sounds, as increasing the excretion of uric acid can help alleviate the symptoms of gout. Celery also has vitamin C and beta-carotene, which may help prevent gout. Celery is easily consumed as a shake, or check out these great recipes if youre hurting for ideas.

Health Conditions Responsible For Gout Flare Ups:

Sometimes, stress due to health conditions like pneumonia or medical procedures such as visiting the hospital or undergoing a surgery may also increase your uric acid levels. If you are visiting the doctor or have fallen sick or are going to the hospital for a surgery, make sure that you tell your doctor that you have gout.

I Have Read That Water Can Help Prevent Flare Ups Is There Any Reason Why Any Research To Back It Up Or Is It Just A Myth

Water acts as an apparatus that uric acid travels in during the elimination process by your body. During a gout attack it’s important to work towards lowering uric acid levels as the continued accumulation of them causes the creation of monosodium urate crystals that cause the painful inflammation and swelling of gout. Decreasing your uric acid levels can help manage the pain and can also decrease the duration of your gout attack by essentially ridding of the ‘fuel causing the fire’.

Think of this process like a train: the uric acid or passengers collect in the urine or train and when signaled, the urine leaves your body when you relieve yourself. When you don’t drink enough water, there aren’t enough trains for the passengers , so gradually the uric acid begins to accumulate and when that occurs it undergoes its crystallization process that ultimately leads to a gout attack.

    How Does Diet Impact Gout

Though diet isnt the only factor that leads to gout, it can play a role. To know why, its important to understand what causes gout in the body .

Gout occurs when an excess of uric acid in the body crystallizes in the joints, most commonly in the big toes, fingers, wrists and knees. These sharp crystals cause sudden swelling and are the source of the pain associated with a gout attack.

Uric acid is produced naturally when the body breaks down purines. Most purine is produced inside the body, though purines are also contained in some foods. Normally, uric acid doesnt cause problems because the body is able to secrete it easily through urine. However, some people produce too much uric acid or are unable to secrete enough of it. In these cases, the result is gout.

Though the body produces uric acid naturally, making gout largely unavoidable, consumption of foods rich in purines can exacerbate the problem. In fact, one study found that high purine intake was associated with five times greater risk of recurrent gout attacks .

In addition to foods rich in purine, those with gout should also avoid foods with lots of added sugars and refined carbs. Though more research needs to be done, reducing glycemic index seems to correlate with lower uric acid levels .

Foods That Cause Gout Flare Ups

The development of gout is influenced by several uncontrollable factors like genetics and age, which means the best way to prevent the swelling and intense pain of a gout attack is to work with a doctor regularly. However, some factors that contribute to gout, such as diet and weight, are more under an individuals control.

Because suffering from gout can be so painful, many people may want to take measures to prevent flare-ups as much as possible. Luckily, by avoiding certain gout-inducing foods in addition to taking medication and following a doctors recommendations its possible to prevent some attacks.
