Which Joints Are Involved In Gouty Arthritis And Why Is It Most Common In The Foot
As with all other known types of arthritis, Gout has particular joints it tends to attack, and the foot is its most common location. Gout especially favors the bunion joint, known as the first metatarsophalangeal joint , but the ankle, midfoot and knee are also common locations, as is the bursa that overlies the elbow.
The bunion joint is the first joint involved in 75% of patients and is ultimately involved in over 90% of those with this condition. . It is thought that this joint is especially involved in gout because it is the joint that receives the highest pounds per square inch of pressure when walking or running.
Late in gout, if untreated, multiple joints can be involved, including the fingers and wrists. The shoulder joint is very rarely involved by gout and the same is true of the hip.

Walking and standing are almost impossible if the legs or feet are affected. Many patients have flu-like symptoms, including fever and chills. The pain may go away on its own in a few hours, or it may take a few weeks.

Signs And Symptoms Of Gout
Any joint can be affected by gout, but it usually affects joints towards the ends of the limbs, such as the toes, ankles, knees and fingers.
Signs and symptoms of gout include:
- severe pain in one or more joints
- the joint feeling hot and very tender
- swelling in and around the affected joint
- red, shiny skin over the affected joint
Symptoms develop rapidly over a few hours and typically last three to 10 days. After this time the pain should pass and the joint should return to normal.

How Gout Treatment Prevents Disease Progression
Fortunately, in part because of its long history, gout is one of the most well-understood and most medically treatable forms of arthritis. The progression of gout is preventable by starting appropriate treatment as soon as possible, Dr. Meysami says. Uric acid-lowering agents, such as allopurinol , can decrease uric acid levels and prevent gout attacks.
During flares, gout patients can take NSAIDs or corticosteroids to calm the attack. Local steroid injections may also be given. A medication called colchicine is also often used to stop acute attacks, but its given now at a much lower dose in order to avoid side effects, including stomach upset.
After a flare ends, long-term treatment can begin. Almost all people with gout will need medication to control their urate, Dr. Fields says. To get a good outcome, people with gout need to get their blood urate level below 6.0 mg/dl and keep it there. By doing that, the body will gradually pull the urate crystals out of their joints, and the flares can stop. Tophi will also gradually disappear.
Medications called xanthine oxidase inhibitors limit the amount of uric acid your body produces. These include allopurinol and febuxostat . Colchicine, which decreases the inflammatory response to gout, may also be given along with allopurinol at the start of long-term treatment after a flare subsides.

So youve had about three months without pain? No loss of joint mobility? Just constant slight swelling? Odd. Certainly not something Ive experienced so Im kind of at a lossI think youd better get that foot examined by someone who knows about many different types of joint or foot problems, not just about gout. You didnt say if you ever got a solid gout diagnosis. And its possible a gout attack did some damage and that the problem you have now is related to but separate from gout.But whatever the cause of your lingering swelling, if you cant or wont try rest and possibly physical therapy to help with the healing, Im guessing infiltrating or injecting a drug that fights inflammation in your foot might help without as many side effects as youd get from taking a similarly nasty drug in pill form. But Im no doctor and Ive not even seen your foot so thats obviously a really wild guess.There are gentler things some of us have tried to get rid of lingering symptoms and help to heal along but three months? I think youre past home remedies and the like.Your symptoms might not seem like a big deal right now but it might possibly the early signs of something that could get worse over the years. Best see a qualified professional and catch that kind of thing early.

Know Which Foods Cause Gout Attacks
Third, whether you are taking medicine or not, you will have to make changes in your diet in order to reduce the frequency and severity of attacks. You should learn and note which items in your diet cause flare-ups, and avoid foods that cause gout symptoms to manifest. Avoiding these foods that cause gout is far easier than enduring another flare up.

Red Skin On Gout Area But No Pain
Does anyone else have this issue? The skin on top of my right foot on and near the gout area on my big toe gets very red from time to time. I notice that it is extra red when I take off my socks or after wearing house slippers. Is this a minor attack? The affected area does not swell up like a normal gout attack and there is no pain.
I had my first gout attack back in early September 2017 and did not start on UA medication until November. My last major attack was back in early December and have not had a major one yet . My UA level last time I checked in December was 4 mg/dl.
I used to get little patches of skin redness on a regular basis.If you dont have much UA in your blood, small deposits which come in direct contact with the blood could melt away before they have a chance to cause swelling and pain.If your skin redness is gout-related, considering your UA level it should stop occurring before long.

When Is Surgery Considered For Gout
The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone. Whenever possible, however, we try to avoid surgery to remove tophi. The problem is that the crystals are often extensive, and track back to the bone, so there is not a good healing surface once the tophus is removed. In some rare cases, such as when a tophus is infected or when its location is causing major disability, surgical removal may be considered.
Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase , since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus.
How Is Gout Treated
Medicines can help treat gout. To stop a gout attack, your doctor will prescribe pills such as colchicine or NSAIDs . Sometimes it’s treated with a steroid shot. The sooner you get started on medicines, the sooner your symptoms will start to get better.
Resting the joint that hurts can also help you feel better. Taking ibuprofen or another anti-inflammatory medicine can help too. Using ice on the sore joint may also help.
To manage long-term gout and prevent future attacks, your doctor can prescribe a medicine to reduce uric acid buildup in your blood. For the first few months you will probably take another medicine with it to prevent attacks until the long-term medicine has time to work.

How To Know If Your Gout Is Progressing
As you become more familiar with gout symptoms, you may be able to sense that a gout attack is coming on. Worsening of pain, swelling, redness, and warmth of the affected joint during the attack is the sign of progression of that attack, Dr. Meysami says.
In addition, the disease overall may progress with recurrent or more frequent gout attacks with longer duration, the involvement of more joints, and the presence of tophi, Dr. Meysami says.
