How To Take It
Swallow the allopurinol tablets with water, ideally after food. You’ll usually take it once a day, but if you’re on a high dose, your doctor may advise you to split the dose and take it twice a day.
If your doctor has recommended you take allopurinol with lots of fluid, try to drink 2 to 3 litres of fluids every day.

Dosage For Treating Gout
Your doctor will start you on a low dosage of allopurinol oral tablets for gout. This is typically 100 mg once daily.
After a week, your doctor may order a blood test to check your uric acid level. If its still high, theyll usually increase your dosage to 200 mg per day.
Your doctor will continue this pattern of checking your uric acid level and increasing your dosage once per week. When you reach the dosage that manages your uric acid level effectively, youll likely continue taking this dosage long term. You should keep taking your usual dosage during gout attacks.
For most people, the usual long-term dosage for gout is 200 mg to 600 mg per day. Your doctor will advise you on whether to take your dose once daily or split it up into two doses per day. The maximum daily dose of allopurinol is 800 mg.
Adjusted dosage for kidney disease
Your doctor may recommend a different dosage depending on several factors. This includes whether you have renal disease and the severity of it. Theyll typically prescribe a lower dosage of allopurinol than usual if you have kidney disease. This is called renal dosing.

What Is Allopurinol Gout Treatment
Allopurinol is a common treatment for gout used to block excess production of uric acid and help the body excrete acid that might cause problems. It is a xanthine oxidase inhibitor, which means taken regularly, allopurinol can prevent recurrence of gout attacks. Allopurinol gout treatment helps break down purines in uric acid that can form crystals around joints. The crystals can settle in the soft tissue surrounding the joint, provoking irritation that leads to swelling and pain.
About one-fifth of purines come from food. The rest is made in the body and excreted in the urine or feces. When production of uric acid increases or does not leave the body, allopurinol gout treatment can help.
Most patients begin with 100 mg a day, with the dosage increased until uric acid drops to acceptable levels. The medicine can be adjusted up to a maximum 800 mg a day. Once allopurinol treatment begins, it should continue every day to prevent further gout attacks, and it might take six month to a year before episodes cease.

How Can A Gout Attack Be Prevented

As with all other known types of arthritis, Gout has particular joints it tends to attack, and the foot is its most common location. Gout especially favors the bunion joint, known as the first metatarsophalangeal joint , but the ankle, midfoot and knee are also common locations, as is the bursa that overlies the elbow.
The bunion joint is the first joint involved in 75% of patients and is ultimately involved in over 90% of those with this condition. . It is thought that this joint is especially involved in gout because it is the joint that receives the highest pounds per square inch of pressure when walking or running.

When Is Surgery Considered For Gout
The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone. Whenever possible, however, we try to avoid surgery to remove tophi. The problem is that the crystals are often extensive, and track back to the bone, so there is not a good healing surface once the tophus is removed. In some rare cases, such as when a tophus is infected or when its location is causing major disability, surgical removal may be considered.
Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase , since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus.
When Dosed Appropriately Allopurinol Is Not Inferior To Febuxostat In The Treatment Of Gout
Allopurinol is often recommended over febuxostat for gout patients when starting uric acid-lowering therapy but is one better than the other in the management of gout?
A team of researchers from University of Nebraska Medical Center and Boston University School of Medicine, among others, set out to find the answer in a 72-week trial done in the veterans hospital and funded by the Veterans Association . It included 950 patients who were randomly given allopurinol or febuxostat to lower serum urate levels.
The results: Allopurinol was not inferior to febuxostat and produced similar flare rates for both patients with and without significant chronic kidney disease . Overall, 80 percent of patients achieved target uric acid levels and nearly 90 percent achieved levels below 6.8 mg/dL, with no difference according to uric-lowering treatment. There was also no difference in patients with CKD.
This trial demonstrates that titrating urate-lowering therapies to serum urate targets can reduce important clinical outcomes of gout flares, and that allopurinol, when titrated to doses greater than 300 mg, works about as well as febuxostat, which is a more expensive medication, Boston University rheumatologist Jean Liew, MD, told CreakyJoints.
Thats an important finding since allopurinol is 19 times cheaper than febuxostat, at least at the VA, and because of the black box warning , the studys lead author James ODell, MD, said in a RheumNow video.

Along with diet, physical activity can help with weight loss, and gout has been associated with being overweight.7 in patients with well-established gout, especially if X-rays have demonstrated joint damage in the foot, a low-impact exercise program is reasonable.
