The goals of treatment of gout are to reduce the pain associated with acute attacks, to prevent future attacks from occurring, and to avoid the formation of tophi and kidney stones.Your physician may recommend one or more of the following treatments for acute attacks:

  • Rest and elevation of the affected joint
  • Anti-inflammatory medications, including ibuprofen, naproxen, or indomethacin
  • Colchicine
  • Corticosteroids such as triamcinolone injections or prednisone pills

  • Allopurinol
  • Probenecid

How Will Gout Affect Me

Attacks can vary from person to person. Some people only have an attack every few years, while others have attacks every few months.

Without medication attacks tend to happen more often and other joints can become affected.

Having high urate levels and gout for a long time can lead to other health problems, including:

  • narrowing of the arteries – which can lead to an increased risk of stroke or heart attacks or other heart problems
  • osteoarthritis, which occurs when the urate crystals and hard tophi cause joint damage.
  • an increased risk of developing kidney disease or worsening of the condition if you already have it
  • kidney stones
  • an increased risk of some cancers, especially prostate cancer
  • mental health problems, including depression
  • underactive thyroid

    What Causes Arthritis Of The Elbow

    Rheumatoid arthritis is a disease of the joint linings, or synovia. As the joint lining swells, the joint space narrows. The disease gradually destroys the bones and soft tissues. Usually, RA affects both elbows, as well other joints such as the hand, wrist and shoulder.

    Osteoarthritis affects the cushioning cartilage on the ends of the bones that enables them to move smoothly in the joint. As the cartilage is destroyed, the bones begin to rub against each other. Loose fragments within the joint may accelerate degeneration.

    ii. Allopurinol: This agent is presently the most commonly used drug for the prevention of gout. Allopurinol blocks the enzyme xanthine oxidase, which blocks the breakdown of purines, thus decreasing the bodys total amount of uric acid. Allopurinol is effective in preventing gout no matter what the mechanism of the elevated uric acid was. Whether a person is making too much uric acid, or has difficulty excreting it via the kidney, allopurinols decrease in uric acid production leads to the same goal: a decreased total body uric acid.

    What Are The Causes And Triggers Of Gout In The Elbow

    The buildup of uric acid in the body is known as hyperuricemia. Your body produces uric acid when it breaks down purines. These are compounds found in all your cells. You can also find purines in several types of food, especially red meat and some seafood, as well as alcohol and some sugar-sweetened drinks.

    Usually, uric acid passes through your kidneys, which help to eliminate extra uric acid in your urine. But sometimes theres too much uric acid for your kidneys to handle. In other cases, the kidneys cant process a typical amount of uric acid due to an underlying condition.

    As a result, more uric acid circulates throughout your body, ending up in your elbow as uric acid crystals.

    While some gout attacks will solve quickly by themselves, the majority will go on for a week, several weeks, or even longer if not treated.     Considered a type of arthritis, gout is very common. It affects nearly 10 million U.S. adults, according to a 2019 study in the journal Arthritis & Rheumatology. However, the thumb is quite an unusual place for gout to make its first appearance, Dr. Fields explains.

    He says gout can occur in any joint in the body, but the most common spot for a first flare of gout is at the bunion joint of the big toe, followed by the mid-foot, then the ankle and the knee.

    Theres some thought that gout in the lower extremity, and especially the big toe, may happen earlier than in other joints because of pressure from walking, Dr. Fields explains. In the first toe, it has been postulated that it being the coolest part of the body, you get more gout since we know that gout crystals are more likely to come out of solution in cooler temperatures.

    But gout can develop in any joint in the hand, including the thumb but also in finger joints as well as the wrist, elbow, and even the shoulder. Gout tends to affect more joints in the body the longer you have it and if it is not well-controlled.

    If you experience a gout attack in your thumb, it is likely a sign of advancing gout, with previous flares occurring in other joints such as your big toe. If you suspect you have gout symptoms in your thumb, fingers, or hand you should seek medical attention.

    How Can An Attack Of Gout Be Treated

    The management of an acute attack of gout is very different from the prevention of subsequent attacks.

    Treatments used for prevention, such as allopurinol can actually make things worse if given during an attack, and so need to be held back until the attack has resolved for several weeks.

    There are a number of measures that can help resolve an attack of gout. See Table 2 for summary of treatment strategies for acute gout. One principle is that treatment for an attack of gout should be instituted quickly, since quick treatment can often be rewarded with a quick improvement.

    If an attack of gout is allowed to last more than a day or so before treatment is started, the response to treatment may be much slower.

  • Nonsteroidal anti-inflammatory drugs or COX-2 inhibitorsExamples of : Naproxen 500mg twice daily, indomethacin 25mg three times daily. Example of COX-2 inhibitor: celecoxib 200mg twice a day. Possible side-effects: Elevation of blood pressure, ankle swelling, upset stomach, ulcer . Use with caution if kidney or liver problems.
  • Anti-Inflammatory corticosteroidsExamples of : Prednisone 40mg first day, 30mg 2nd day, 20mg third day, 10mg fourth day. Possible side-effects: Elevation of blood pressure, elevation of blood sugar, mood changes. Short-term use, as in gout, generally much better tolerated than long-term use. Use with caution if diabetic.
  • Get An Accurate Diagnosis And Treatment

    While gout attack symptoms do resolve on their own, people are advised to seek medical treatment. If left untreated, gout can become chronic and cause lasting joint damage.

    Diagnosis begins with a clinical exam and interview. Patients are encouraged to be honest with their health care providers about their eating, drinking, and other lifestyle habits. This honesty can be helpful both for diagnosing gout and creating a treatment plan. Diagnosis may also involve blood tests, urine tests, and medical imaging, such as x-rays and ultrasound.

    Treatment typically involves advice to avoid alcohol and certain foods known to trigger gout, such as foods and drink high in sugar, seafood, red meat, and organ meats.19 Medications to lower urate levels in the bloodâwhich can lead to gout attacksâare often recommended. Losing excess weight is another effective way to reduce the risk of a gout attack and chronic gout.20

    Medications For Acute Gout

  • Non-steroidal anti-inflammatory agents and COX-2 inhibitors are the mainstay of therapy of acute attacks of gout in patients who have no contra-indication to them. These medications include such agents as naproxen , ibuprofen , celecoxib , indomethacin and many others. These agents reliably decrease the inflammation and pain of gout. However, patients with ulcers, hypertension, coronary disease, and fluid retention must be careful with these agents, even for the short courses needed to resolve a gout attack. The doses of non-steroidal anti-inflammatory agents needed to resolve a gout attack are on the higher side, since full anti-inflammatory effect is needed. See examples of dosage in Table 2. Over-the-counter dosage levels, for example, ibuprofen at 200mg, two tabs three times a day, are often insufficient.
  • Corticosteroids, such as prednisone and methylprednisolone , are anti-inflammatory agents that are quite effective against gout attacks. Anti-inflammatory steroids are very different in action and side-effects as compared to male hormone steroids. Anti-inflammatory steroids have long-term risks, such as bone thinning and infection, but their risk for short-term therapy is relatively low. These agents can raise blood pressure and blood sugar, so can be a problem for those with uncontrolled hypertension or uncontrolled diabetes mellitus.
